Achenbach ASEBA Preschool Ages 1.5 – 5 years: ASEBA – Pre-School Module Ages 1.5-5 -The Child Behavior Checklist (CBCL/1.5-5) and the Caregiver-Teacher Report Form (C-TRF), revised in 2000, are a set of rating forms and profiles for the preschool-aged child. They replace the legacy (previous) editions of CBCL/2-3 and C-TRF/2-5. The profiles for the two instruments have the following 6 cross-informant syndromes: Emotionally Reactive, Anxious/Depressed, Somatic Complaints, Withdrawn, Attention Problems, Aggressive Behavior. The CBCL/1.5-5 also has a Sleep Problems syndrome; while both forms have parallel Internalizing, Externalizing, and Total Problems scales.

Child Behavior Checklist (CBCL) – 1.5 – 5 YEARS (side 1)- The CBCL/1.5-5 obtains parents’ ratings of 99 problem items; plus open-ended questions asking for descriptions of problems, disabilities, what concerns parents most about their child, and the best things about the child. Note: The legacy editions of CBCL/2-3 and C-TRF/2-5 can be scored using the new profiles, but is strongly recommended that all new assessments of pre-school children employ the new editions.

CBCL 1.5-5 DSM-Oriented Scales for Boys and Girls ( side 2 – found on the back of the CBCL1.5-5 form) – The preschool profiles feature DSM-oriented scales in addition to empirically based scales. Scales were constructed for the following 5 DSM-oriented categories: Affective Problems, Anxiety Problems, Attention Deficit/Hyperactivity Problems, Oppositional Defiant Problems, and Pervasive Developmental Problems.The DSM-oriented scales can also be scored by hand on profiles printed on the back of the hand-scored profiles for the syndrome scales. The DSM-oriented scales serve as supplements to the empirically based scales for users who wish to view item scores in relation to DSM categories as well as in relation to empirically based syndromes.

Caregiver-Teacher Report Form For Ages 1.5 – 5 – (side 1- Empirically Based/ side 2 – DSM-Oriented) The C-TRF obtains ratings from daycare providers and teachers on 99 items, plus descriptions of problems, disabilities, what concerns the respondent most about the child, and the best things about the child. Language Development Survey (LDS) for Ages 18 months – 35 months – An important feature of the CBCL/1.5-5 is the LDS, which uses parents’ reports to assess children’s expressive vocabularies and word combinations, as well as risk factors for language delays. The LDS indicates whether a child’s vocabulary and word combinations are delayed relative to norms for ages 18-35 months. The LDS can be completed for language-delayed older children for comparison with norms up to 35 months.
